Hyperglycemic crisis under which are Diabetic Ketoacidosis (DKA) and Hyperosmolar Hyperglycemic State (HHS) are the two most serious acute metabolic complications of diabetes. Despite advances in the prevention and management of diabetes, its prevalence and associated health care costs continue to increase worldwide. However, in institutions like BGHMC, the degree of physician compliance with these guidelines and its impact on patient recovery has not been systematically assessed. This study aims to evaluate the compliance of physicians with hyperglycemic crisis management protocols and determine the prevalence of adherence to the standard of care in relation to clinical outcomes. Aim: To evaluate the compliance rate to the 2009 American Diabetes Association guideline in managing hyperglycemic crises, cause and its clinical outcome among adult patients diagnosed with hyperglycemic crises at Baguio General Hospital and Medical Center from January 2023 to December 2024.
Cross sectional study was conducted on patient records adults age >18 diagnosed with hyperglycemic crises at the emergency room of Baguio General Hospital and Medical Center from January 2023 to December 2024. Clinical profiles of participants will be described in a frequency table.
A total of 73 adult patients who were admitted due to hyperglycemic crises were enrolled in the study. The majority were middle aged patients (40-60 years old). Not all patients had co-morbid conditions however hypertension was the most prevalent in both groups. Most patients also had diabetes for less than 10 years and half of them had no complications of diabetes. Physicians were partially compliant to the guideline set by the American Diabetes Association and still the leading cause of hyperglycemic crisis includes infection particularly pneumonia and non compliance to medications. Patients admitted for hyperglycemic crises at Baguio General Hospital and Medical Center demonstrated favorable clinical outcomes.
The study underscores the critical role of adherence to the 2009 ADA guidelines in the management of hyperglycemic crises, as compliance is associated with improved clinical outcomes.
